I know exactly what you’re talking about. I also enjoy making lots of jokes. But sometimes I go too far and end up hurting people’s feelings.
Well, at least what you’re expressing right now shows that you’re a human being and not some psychopath. It’ll get better. They’ll understand that you didn’t mean what you said.

I spend so much time apologizing for stuff I thought was funny but turned out only to be hurtful, I ought to know better by now … yet somehow, I don’t.

If the person you hurt is your friend, I’m sure she will understand that you never meant to hurt her. There are those who are careless and hurt others’ feelings without realising or worrying about it. But most of us do it unintentially every once in a while. I forgive easily if I can see that the person is genuinely sorry and meant no harm.
